Guidelines for the Assessment for the Implementation Tool

· Identify the interprofessional team members critical to the change implementation
· Summarize how the evidence-based change project was presented to the interprofessional team,
· Detail how the project was presented to the interprofessional team
o Details should include the successful method used for the presentation to the interprofessional team
· Describe how the meetings helped to facilitate the implementation process, what could have been done differently and why
· Identify concerns that the interprofessional team raised
· List what the barriers were to the implementation of the evidence-based change project
o How were the barriers remedied
· During the process of implementation, were other unforeseen barriers identified and if so, how were they addressed
· Reflect on how barriers could have been minimized or removed
· Review the implementation process and include a discussion of the successes and challenges
· Discuss how your initial implementation plans compared to the actual implementation process
o Where the original plans utilized? if not describe why change was needed
o Were the plans successful
· Discuss any changes that would be recommended after a review of the evidence-based change project upon completion,
· Detail what helped
· Discuss what resources were needed.
o Provide a general overview of what else may have helped to better support the success of the project
