each subject matter.
To make reports more unified the following template
provides general guidelines for what should be included in each
section.

Summary should include all these areas of
information. Use headers to organize
information.
o
Education/Development: Are they in school? What grade and where (if
the information isn’t confidential)? Are
they at grade level? Do they receive any services (ie. IEP, 504, reading
assistance). For non-school kids- are
they developmentally on target? Give examples (walking, talking). Are there any eating or sleeping issues? Are
there any concerns for drug exposure for infants? Have any assessments been done? Are they in speech therapy?
o
Medical/Dental:
Please do not just state “up to date on medical/dental.” Provide date and provider of last medical and
dental appointments and outcomes of those appointments. List upcoming appointments with dates and
provider names. Are there any health
issues? Do they wear glasses or braces? Are they in occupational or physical
therapy and for what?
o
Social
Skills/Relationships: How do they
get along with peers? Adults? Caregivers? Siblings? Do they need help with social skills?
o
Psychological/Mental
Health: Are they in therapy- where and how often? What is their diagnosis? What are they
working on in therapy? Are they making
any progress?
o
Ethnic/Cultural
Needs: Are we meeting their ethnic or cultural needs? Are they placed with
family who can meet these needs? Is the
foster parent providing them with opportunities to experience their culture? Have you provided information for cultural
resources and events?
o
Visits:
How often is the child visiting with the parents? Siblings? How does the child
respond during visits? Are there any
concerns?
o
Permanency:
What is your recommendation and why?
